AD 2035 Interested persons use this form to sign up for Minority Farm Register in order to receive outreach information regarding FSA programs.
CCC 10 Applicants use this form to make certain representations to CCC to obtain loans. For FSA Farm Loan Programs it is used by an individual who has not signed a FSA-410-1, pledges security for a loan.
CCC 20 Contains the required certification and standard clauses applicable to warehouse operators with a storage agreement to store commodities for CCC.
CCC 21 This form is used by warehouse operators to declare compliance with highly erodible land and wetland conservation provisions of the 1985 Food Security Act as amended.
CCC 22 This Agreement defines terms between the warehouse operator storing and handling of CCC-owned peanuts or pea. pledged as collateral for a CCC marketing assistance loan.
CCC 22-1 Warehouse operators use this form when applying for a Peanut Storage Agreement to store Commodity Credit Corporation (CCC) owned and loan peanuts.
CCC 24 Warehouse Operators use this form when applying for a Uniform Grain and Rice Storage Agreement to store CCC-owned and loan grain.

CCC 36 Assignment of Payment (09-09-09): Used by producers to assign payments under various Commodity Credit Corporation or Farm Service Agency program. Farm loans, farm storage facility loans, and purchase agreement proceeds are not eligible for assignments.
CCC 37 Joint Payment Authorization (09-09-09): Used by producers to make payments from the CCC or FSA jointly payable to multiple entities.
CCC 38 Producer must read this form to get an overview of what payment options are available to them as a recipient of a CCC or FSA payment
CCC 49 Warehouse operators use this form when applying for a Cotton Storage Agreement to store CCC-owned and loan cotton.
CCC 100 A company that imports unmanufactured tobacco into the United States uses this worksheet to calculate the amount of Importer No Net Cost (INNC) Assessment due on the total of imported tobacco.
CCC 126 Warehouse operators use this form to make specific changes to the original Warehouse Operator Bond.
CCC 185 Loan Application and Approved for Farm Storage and Drying Equipment, revised 08-17-2009. Provides an application for FSFLP
CCC 186-SU The FSA office responsible for processing your sugar related facility loan completes this form.
CCC 190 Used to obtain waivers of liens that have been filed on collateral by parties other than CCC.
CCC 191 County Offices obtain the CCC-191 from the primary contractor(s) for storage and handling facility construction projects to protect CCC from mechanics lien.
CCC 193-D The FSA office responsible for processing your Farm Storage Facility loan completes this form.
CCC 194 Farm Storage Facility Loan Subordination Agreement (Lien on Real Property) This form is used to determine eligibility for CCC financing for farm storage and drying equipment.
CCC 279 Promissory Note. This form can be used by producers or debtors who request to pay off their debt(s) through an Installment Agreement, and that request has been approved by the respective County or State FSA Office that has the authority to approve such a request.
CCC 296 Used by County Offices when certifying an attorney who provides legal services for the applicant such as preparing a title opinion, handling the loan closing, etc.
CCC 297 Provides a serverance agreement for FSFLP to be used when there are real estate lien holders involving the land where a storgae structure is to be located.
CCC 299 Used to obtain information regarding holders of real estate liens on land where a storage facility will be located.
CCC 330-330-A Agreement entered into by 501(c)(3) organizations to order donated CCC-owned instantized nonfat dry milk & products to be exchanged for CCC-owned nonfat dry milk to help feed the hungry in the U.S.A
CCC 334 This Agreement is entered into by the Feed Dealer and CCC for sales of nonfat dry milk (NDM).
CCC 370 This form is an application used by producers to determine eligibility for Livestock Compensation Program (LCP) and to calculate the amount for payment.
CCC 400 This form is used to allow the conveyance of Farm Storage Facility Loan Program collateral to CCC for sale.
CCC 434 This form is used by owners of trees that produce an agricultural commodity and have been lost as a result of a natural disaster.
CCC 441 Income Used by FSA C.O. to record producers certification of income eligibility. The cert. is maintained in the FSA C.O. as evidence that the producer claims to meet the statutory and regulatory requirement
CCC 452 Manual NAP Actual Production History and Approved Yield Record. Used by C.O. to record crop appraisals and measured production.
CCC 453 Form CCC-453, AILFP Contract to Participate, is to be used by American Indian tribal governments to request participation in AILFP.
CCC 490 (2004) Producers use this form when applying for certified seed incentive payment and for production incentive payment.
CCC 490 (2005) This form is used by Producers when applying for certified seed incentive payment and for production incentive payment.
CCC 491 Beneficial Interest Certification for Loan and Loan Deficiency Payments (11-06-09): Producer submit CCC-491 to certify that they have beneficial interest in a commodity for which a loan or LDP is requested
CCC 501-A Producers use this form to report information about their farming operation that is used by FSA to determine the structure of entities for payment purposes.

CCC 502 Cont Continuation Sheet for Leased or Owned Land (Use with CCC-502A, CCC-502B, CCC-502C, CCC-502D, CCC-502EZ)
CCC 502-A Farm Operating Plan for Payment Eligibility Review for an Individual. This form is used to collect information about producers to determine their eligibility for payments from FSA.
CCC 502-B This form is used to collect information about joint ventures and general partnerships that is used by FSA to determine eligibility for payments.
CCC 502-D Farm Operating Plan for Payment Eligibility Review for an Estate or Trust
CCC 502-EZ Used to collect information about individuals to determine eligibility for payments. Abbreviated version of payment eligibility review.
CCC 502-U Simplified form used to update data that has been changed since the previous year
CCC 505 Voluntary Permanent Direct and Counter-Cyclical Program (DCP) Base Acreage Reduction. This form is used by owners who enroll all or a portion of their base acreage prior to enrollment in DCP.
CCC 509 (04-16-09) Direct and Counter-Cyclical Program (DCP) Contract and Average Crop Revenue Election (ACRE) Contract dated 04-16-09. This form serves as an annual contract between producers and CCC.
CCC 509 (06-20-08) 2008 Direct and Counter-Cyclical Program Contract dated 06-20-08. This form serves as an annual contract between producers and the Commodity Credit Corporation. Producers who participate in the program must fully comply with the terms of the contract, and in return will receive direct payments and counter-cyclical payments, if such payments are authorized.
CCC 509 (10-01-09) Direct and Counter-Cyclical Program (DCP) Contract and Average Crop Revenue Election (ACRE) Contract dated 10-01-09. This form serves as a annual contract between producers and CCC.
CCC 509 ACRE Average Crop Revenue This form is Average Crop Revenue Election (ACRE) Program Irrevocable Election.
CCC 509 Appendix (04-24-09) Appendix to Form CCC-509, Direct and Counter-Cyclical Program (DCP) Contract or Average Crop Revenue Election (ACRE) Program Contract dated 04-24-09. (2009-2012). This information is necessary for CCC to consider and approve the contract to enter into the DCP or ACRE program, to assist in determining eligibility, and to determine the correct producers on the contract.
CCC 510 DCP or ACRE Program Cash Rent Producers who case rent land may certify provisions of their cash lease.
CCC 517 This form is used by tract owners who sign the tract distribution form agreeing to the redistribution of base acreage for each tract on the farm based on the method selected.
CCC 523 Summary of Acreage History and Yield Report. Furn to farm oper and owner when FSA records indicate a history of purse crops from 1998-2001.
CCC 526 (08-12-08) 2008 Payment Eligibility Average Adjusted Gross Income Certification dated 08-12-08. Producers use this form to certify compliance with the AGI provision for the receipt of commodity, conservation, and price support program benefits.
CCC 526 (10-15-03) Payment Eligibility Average Adjusted Gross Income Certification dated 10-15-03. Producers use this form to certify compliance with the AGI provision for the receipt of commodity, conservation, and price support program benefits.
CCC 526-C Payment Eligibility – Average Adjusted Gross Income Certification For Certain Conservation Reserve Program Contracts Approved Before October 1, 2008
CCC 528 Designation of Payment Acres for Planted and Considered Planted (P&CP) Crops of Covered Commodities and Peanuts for the Average Crop Revenue Election (ACRE) Program, dated 08-20-09. This form is used for CCC to determine the order of paymetn of covered commodities and peanuts on farms that are participating in ACRE when P&CP acres of covered commodities and peanuts on the farm exceed base acres.
CCC 564 The information provided by the producers is used for applying for the 2001 or 2002 crop disaster program. The County Office will assist producers in completing this form.
CCC 564-M This form will only be used for producers applying for the 2001 or 2002 crop disaster program that have applied for benefits on acreage that has been planted to multiple crops.
CCC 565 This certification must be completed by each applicant for disaster assistance or an authorized representative of the applicant.
CCC 566 This information will be used to determine eligibility for program benefits on an insurable crop that is not insured, or on a noninsurable crop that is not covered by NAP.
CCC 567 This form will be used by producers for applying for the 2001 and 2002 Sugar Beet Disaster Program.
CCC 576 Producers use this form to request NAP assistance when natural disasters cause a catastrophic loss of production or prevented planting of an eligible crop produced for food or fiber, CAT not available
CCC 576-1 This form is used by FSA county office to record crop appraisals and measured production. It is used in conjunction with the CCC-576 to calculate unit production.
CCC 577 Producers use this form to request a transfer of NAP coverage when a change of share or sale of covered crop acreage occurs during the coverage period.
CCC 580 The MILC program is available to producers on dairy operations throughout the U.S. if the dairy operation produces and commerically markets milk during the period of Dec. 1, 2001 – Sept. 30, 2005.
CCC 580 Addendum 1 Notifies producers to the modifications to CCC-580 Appendix made in accordance with MILC regs to accept modified terms and conditions of CCC-580 Appendix in contract addendum.
CCC 580 Appendix The MILC program is available to producers on dairy operations throughout the U.S. if the dairy operation produces and commerically markets milk during the period of Dec. 1, 2001 – Sept. 30, 2005.
CCC 580-M MIlk Income Loss Contract Extension (MILC) Modification. Producers use this form to modify an existing approved MILC with CCC.
CCC 580-M SP MILK INCOME LOSS CONTRACT (MILC) MODIFICATION. Producers use this form to modify an existing approved Milk Income Loss Contract with the Commodity Credit Corporation (CCC).
CCC 580-S Milk Income Loss Contract (MILC) Supplemental. Used to supplement their application to enter into a MILC with CCC and provide information about the operation used by FSA for determination.
CCC Apendice del CCC-580 The MILC program is available to producers on dairy operations throughout the U.S. if the dairy operation produces and commercially markets milk during the period of 12/01/2002 through 09/30/2005.
CCC 582 Qualified persons, who desire to act as MILC agents to obtain and disburse benefits on behalf of producers will use this form to obtain CCC approval and certify to the terms and conditions.

CCC 605 Auth Electronic Agent and Design of Agent-Cotton. Used to identify persons or companies that may repay CCC the amount due on a producers cotton loan.
CCC 605-P Design. of Agnt Peanuts Includes CCC-605P-1. This form used by peanut producers to designate an agent to redeem all or a portion of their peanut production pledged as collateral for a specified loan.
CCC 605-P-2 Designation of Subsequent Agent – Peanuts This form will be used by peanut producers, designated agents, or subsequent designated agents if less than the loan quantity is designated.
CCC 633-EZ Loan Deficiency Payment (LDP) Agreement and Request, revised 07-31-2009. Used by producers who want to receive a loan deficiency payment. Page 1 must be filed prior to loss of beneficial interest in an eligible commodity. It must be filed before harvest, ginning, shearing, or slaughter if the producer will lose title, control, and risk of loss (beneficial interest) at the time of harvest, shearing, slaughter or delivery. The producer, who would be otherwise eligible for a marketing assistance loan (MAL) must complete the form. One CCC-633 EZ – Page 1 is filed per producer (individual, corporation, partnership, trust, etc.) per crop year. Multiple payment requests can be filed by using Page 2, Page 3, or Page 4 of the CCC-633 EZ, as applicable.
CCC 633 Grazing Grazing Payment Program Application. Used when applying to the Grazing Payment Program when income lost during the time farmers were grazing their livestock.
CCC 633 Honey Honey Nonrecourse Market Assistance Loan Certification and Worksheet This form is used to certify the quantity of honey and the eligibility for a loan.
CCC 633 WM This form will be used by producers of wool and mohair to certify the type, quantity, and identification of production pledged to secure a CCC marketing assistance loan from FSA.
CCC 644 Form CCC-644, AILFP Payment Application, is to be used by producers to apply for AILFP Payments.
CCC 644-A Form CCC-644A, AILFP Continuation Sheet, used by AILFP applicants to record additional feed receipts, feed sold, and livestock when there is no more space left on form CCC-644.
CCC 648 AILFP Region Designation and Feed Loss Assessment, used by tribal governments to record pasture and grazing, forage production, feed grain production, and other roughage lost by natural disaster.
CCC 661 This form is an application for livestock producers when they have suffered significant livestock losses due to weather and other conditions.
CCC 664 Used to authorize the assumption of an existing CCC loan from one entity to another.
CCC 665 Used by a producer who requests a separate CCC loan when the commodity for loan collateral is all or partially commingled in a storage structures with another producers commodity.
CCC 666 Farm Stored Loan Quantity Certification Farm Stored Loan Quantity Certification Worksheet
CCC 679 Lien Waiver Used to document that a Lien Waiver has been authorized by the current lienholder for purposes of pledging the commodity to CCC for a loan.
CCC 681 Authorization to Move Loan Collateral for Rotation or Transfer: Form used by producer to obtain authorization to move loan collateral for rotation or transfer.
CCC 685 Authorization to Release Warehouse Receipts: Producers use this form to request authorization for CCC to release warehouse receipt(s) to a warehouse representative after repayment of a loan quantity.
CCC 686 This form is used when a producer is deceased and has an outstanding loan or loans.
CCC 687-1 Approval to Commingle or Move Loan Collateral. Producers complete who have NOT previously designated additional storage structures on their farm stored grain loan.
CCC 697 Used to lock-in a repayment rate for a limited period of time for the purpose of repaying CCC loan quantity using the locked-in rate. Repayments must be received before the CCC-697 expiration date.
CCC 699C Cotton Transfer Agreement (Includes CCC-699C Attachment (06-25-08) Cotton Transfer Agreement) Used to request the transfer of CCC loan cotton from one warehouse to another.
CCC 732 CMAs or LSAs use this form to request increases in a producers payment limitation.
CCC 736 Addendum CCC-736 (Addendum) is a PA and PB Statement that LSAs complete once to eliminate the need to provide the burden statement on all computer generated CCC forms they create.
CCC 740 Form is used to provide assistance to eligible livestock owners who suffered qualifying feed losses because of natural disaster conditions.
CCC 742 Used by dairy producers to apply for disaster assistance payment program benefits. If you have shares in more than one dairy operation, you must submit separate CCC-742s for each dairy operation.
CCC 743 Used by dairy producers to apply for disaster assistance payment program benefits.
CCC 745 This form is used by dairy producers to certify spoilage losses for the 2005 DDAP-II Program. Producers submit a certification of pounds dumped along with supporting documentation.
CCC 0746 Standard request by CMAs, DMAs, or LSAs for producer eligibility data.
CCC 748 2009 Rice Apportionment: used by producers with rice acreage base to apportion their rice base acres to long grain rice base acres and medium grain rice base acres.
CCC 749 Request to Participate in the __ Planting Transferability Pilot Project: Producers who want to participate in Planting TransferPilot Project on DCP base acre farms in one of 7 eligible states.
CCC 750 Used by producers applying for 2003 – 2005 Crop Disaster (CDP), Virginia Crop Loss (VA-CDP), and North Carolina Fruit and Vegetable Crop Disaster (NC-CDP). County Offices will assist completing.
CCC 750-M Used by the producers to select which crop to receive benifits for when applying for the 2003 – 2005 CDP, VA-CDP or NC-CDP programs on acreage that has been planted with multiple crops.
CCC 751 Determines eligibility for benefits on insurable crops that are not insured, or noninsurable crops that are not covered by NAP. Insurance, or NAP purchase is required for the next two crop years.
CCC 752 Supplemental Agricultural Disaster Assistance Buy-In for Insurable Crops: Allows producers who did not purchase crop insurance coverage for 2008, to be eligible.
CCC 753 2008 Supplemental Agricultural Disaster Assistance Buy-In for Noninsurable Crops: Allow producer who did not purchase NAP coverage for 2008 to be eligible.
CCC 809 Cooperating Ginner’s Bagging and Bale Ties Certification and Agreement. This agreement with CCC and Cotton Ginners provides specifications for approved bagging and bales ties.
CCC 810 Cotton Clerk Application/Agreement. Persons read, complete, and sign this form to apply to become a cotton clerk.
CCC 823 Warehouse operators use this form to enter into a Cotton Storage Agreement to store CCC-interest cotton.

CCC 879 Application for Approval of Cotton Gin Under Seed Cotton Loan Program: Used to determine whether cited gin meets CCC location.
CCC 880 Ginneers Seed Cotton Agreement. Cotton ginners complete this form to certify their agreement with the terms of the recourse Seed Cotton Loan Program.
CCC 883 Cotton producers submit CCC-883 to request authorization to remove seed cotton pledged as loan collateral from storage to sell it or have it ginned.
CCC 894 This appendix is used by sugarcane mills to apply for assistance under the 2005 Louisiana Sugarcane Hurricane Disaster Assistance Program for Louisiana’s sugarcane producers and sugarcane processors.
